- How do I schedule an appointment?
Call our office at (480)964-5800 and ask for scheduling.
- Can I schedule a physical on the first appointment?
Our physicians believe that in order to provide good medical care, that a complete history be obtained. Therefore, we schedule an establish care appointment, where your history is taken, medications you may be taking, reviewed, and if appropriate labs ordered before a complete physical appointment.
- What forms of payment do you accept?
We accept cash, check, Visa and Mastercard. If we are contracted with your Insurance we will submit the claims for you, if you provide us with the correct Insurance information. All co-pays, deductibles and co-insurance are due at the time of visit.
- Do you have evening or weekend hours?
Unfortunately, we do not have any evening or weekend hours. Our hours are M, T, Th, F, 8:00-12:00 noon, 1:00pm - 5:00pm. Wednesday 8:00 - 12:00 noon, 2:00pm 5:00pm.
- How can I obtain my Medical Records?
If you need to request your medical records from a previous physician or specialists, or you need your records forwarded to a health care provider, you can complete a medical records release form and we take care of the request for you. If you want the record for your personal use, you can come to the office and sign a records release and we will copy your record and forward it to you. If your record is going from physician to physician, this is done as a courtesy, but if you want your record for your personal use, there is a $35.00 fee charged per record. Also before any record is sent, your physician will review your records. Due to the extensiveness of this process, your request may take 2-4 weeks. We will expedite this process as quickly as possible.
- Do I need to be seen for a referral?
Yes, if your insurance requires a referral an appointment in our office is necessary for documentation and appropriate diagnosis.
- How can I obtain billing information?
Most of the information used for billing, is found on the patients insurance card. If you have additional questions you can contact our billing service, Next Generation Health Services, at (480) 545-0031. If you need additional help, NGHS will direct you to the appropriate contact.
- Must I pay my co-pay at each visit?
Yes, this is a requirement of your insurance company.
- Do we make payment arrangements?
Payment is expected in full at the time of service.
- Do we have a Spanish speaking physician?
Yes, Dr. Benton and Mary Quihuis speak Spanish fluently.
